There is one log per person, and that person is its custodian. They import it, read it, and decide who else gets to see it. Doctors, hospitals and insurers serve them for a time; the log stays for life. Every other view on this page exists only because that person allowed it. It serves you in good health as much as in illness.
Doctors and providers
